1. Child Wellness Plan
For children through age 9
$225 / year
- 2 dental exams per year
- 2 hygiene visits per year
- 2 fluoride treatments per year
- 2 oral cancer exams per year
- Necessary radiographs at hygiene visits
- 10% savings on restorative treatments (including sealants)
- 20% savings on gingivitis services and adjunct laser therapies
2. Bi-Annual Wellness Plan
For patients 10 years old and up
$495 / year
Family Plan: $495 for first member, $395 for each additional member.
- 2 dental exams & 2 hygiene visits per year
- 2 Guided Biofilm Removals with advanced airflow technology
- 2 fluoride treatments & 2 oral cancer exams
- Necessary 2-D radiographs at hygiene visits
- 10% savings on general restorative (fillings, crowns, endodontics)
- 10% savings on limited/emergency exams
- 20% savings on active periodontal and adjunct laser therapy
3. Quarterly Wellness Plan
For patients 10 years old and up requiring quarterly care
$885 / year
Family Plan: $885 for first member, $785 for each additional member.
- 2 dental exams & 4 hygiene visits per year

- Necessary 2-D radiographs at hygiene visits

* Important Note for New Patients: Plan benefits begin once you are an established patient. A comprehensive initial New Patient Exam and diagnostic x-rays must be completed first to accurately evaluate your oral health, gum tissue, and bone levels prior to plan enrollment. The initial visit is not covered under the membership plan.
* Plan Terms & Flexibility: All plans are based annually but will be honored for a window of up to 18 months from your date of enrollment, ensuring you have ample time to utilize your regular care intervals. Products, cosmetic dentistry, and select surgical procedures are excluded from additional discounts.
